How to Register as a new Patient
When you ask to register you will be given an Application Form to complete. You will be asked to fill out with your personal details, e.g date of birth, previous and current address also, your previous GP. This enables us to request your old records from your previous GP.
This form can also be accessed online in the links below. Please complete the New Patient Registration form and the New Patient Questionnaire.
If you are completing the forms online you will also have to attend the surgery and fill out a new registration GMS form and allow 5 days for your details to be entered on our system before making an appointment.
All new patients (over the age of 5) are encouraged to make an appointment with a Health Care Assistant or Practice Nurse for a simple medical examination. This gives you the opportunity to discuss your past medical history and any current medication. A health questionnaire will be given to you when you join the practice and you are asked to return it completed to the Practice Nurse when you have your health check examination.On-line access consent form

Please complete the following documents, along with the new patient questionnaire and return via email (included in the patient declaration form) to register as a new patient.
Temporary Residents and or Immediately necessary treatment
If you are not registering as a permanent patient, live out of the area and require to be seen by a GP please call the surgery on 01625 861316 and we can advise you.
On booking a temporary resident appointment with a GP or Practice Nurse please attend the surgery at least half an hour before your appointment to complete a temporary resident form and have your information entered into our system.
