How to Register as a new Patient
Welcome to Chelford Surgery, before choosing to register with us, we would like to give you some information about how we work, so you may find our system easier.
We operate a ‘Total Triage System’. This means that we will ask you (or your family member or carer) to use our website to submit a request for a consultation. Our on-call GP will review requests the same day and assess the information to then give advice, either by telephone or in a face-to-face consultation at the surgery. Your appointment may be with a GP or with our Advanced Practitioner, or we may suggest you seek further advice from a pharmacist or another practitioner. Your appointment will be directed to the person deemed most appropriate for your needs and as we are a training practice, it may be with a qualified doctor who is doing their GP specialty training, in a supervised role.
If you do not have access to the internet, you can ask our reception team to complete the form for you or you can fill one in, in person at the surgery.
To register at the practice you will need to complete a new patient registration form.
You can now do this online, without needing to bring in any paperwork. Using the link below to the NHS website, will give you the opportunity to check first if you live within our practice boundary: How to register with Chelford GP surgery – NHS
You will be asked for your name and address, and the name and address of the GP you are registered with at present. Your NHS number if you know it, details of your medical history, any allergies you might have and any medication you are currently taking. You will also be asked for the name and address of your carer if you have one.
This form can also be accessed online and downloaded using this link: New Patient Registration Form
Our Practice information leaflet can be found by clicking here: Practice Leaflet
All new patients (over the age of 5) are encouraged to make an appointment with a Health Care Assistant or Practice Nurse for a simple medical examination. This gives you the opportunity to discuss your past medical history and any current medication.
Temporary Residents and or Immediately necessary treatment
If you are not registering as a permanent patient, live out of the area and require to be seen by a GP please call the surgery on 01625 861316 and we can advise you.
